Thats's terrible. Honestly does make you worry about the NHS

Clearly a number of checks and balances were not followed. I would be breathing fire if this happened to my mum, however lets not forget todays NHS sees and treats considerably more people with considerably less resources and a constant pressure to discharge often with minimal planning and that makes what should be a risk aware service very less risk aware in all the wrong places. Quality care not volume or quantity qoutas!
