Hospital discharge - homefirst

Hi,

Does anyone have any experience of home first? My mum is currently in hospital and been discharged tomorrow on the home first scheme which means within two hours of discharge someone will come to the house and assess her safety and how she will manage.

I’m so worried about my mum being discharged. Before mums hospital admission she was confused but managed to live independently with my dad. Last week she had a collapse and has been in hospital since. We are not sure why this happened as she has refused MRI scans so the hospital won’t do them. She is very very confused now which Drs say may just be progression of dementia and today’s he even tried to eat one of her hearing aids. I am so worried what will happen tomorrow and how she can possibly manage at home. I don’t understand at all what is involved with the homefirst scheme. Has anyone any experience of this service?

Yes & it’s all positive
District nurses who actually get things done in regards to Mum & follow up. lovely folks with years of experience
