Assuming you're UK then the GP and get a referal to the local memory clinic.

Have you thought about trying to get him to a GP to start the ball rolling for a diagnosis. That is normally the first step, then a memory clinic appointment to confirm or otherwise a diagnosis.

The referral can take time so the sooner the better, as much as anything else to eliminate other causes.
