Dad refusing to see a doctor

My 84 year old Dad has been displaying very clear signs of dementia for over 2 years now. The issue is that my dad refuses to accept that he has a problem and appears to be genuinely unaware of the issue leading me to think he may have anosognosia. I have managed to persuade him to go to the doctors on 1 occasion but he refused to engage in any follow up assessment. He has mood swings and while my Mom aged 76 is working hard to care for him she is now very depressed and struggling to cope.

My parents lead an active life and manage to go out and about a lot but my Mom is in a near permanent state of anxiety as she never knows when my Dad’s temper will flare. My dad has no apparent recollection of his outburst although they have been witnessed by a number of people.

Is there any help and support my Mom and Dad could get and if so where from?

Can medical help and support for my dad only come via the GP or can any dementia specialist help?

I would be grateful for any help at all.
