Has anyone experienced an increased amount of errors and omissions on medical records since COVID ‘s arrival
My mum has had a horrendous time medically since the beginning of December including two serious breaks, hip and back and also a “minor “ break of her wrist . She has needed hospitalisation twice and spent two weeks in a care home in the middle.
In all this time information has been sparse for us so we have had to rely on any written reports from her various discharges. The records don’t reflect what little we have been told and in a high number of cases give totally opposite opinions.
Yesterday Mum was discharged to a Nursing home and the report that was sent had opposing information on various issues some serious And some not. Things like E is vegetarian and particularly enjoys meat when offered” we can deal with but we have also found “ E does not have a diagnosis of Dementia” when she entered a specialist nursing dementia home,
We tried to contact her specialist nurse practitioner who “cut and pasted” the report but received no response.
For now mum has been admitted for general nursing care while the “no dementia” wrinkle is ironed out.
