Hi, has anyone recently filled in the COP15DAPed notification and acknowledgment of notification form.
I am unsure about the statement of truth section.
The 1st box to be ticked, I believe ...., does the person notified or I, the applicant tick it?
Is the 2nd box, The applicant...... I am authorised by the applicant to sign this statement. Is this to be ticked if it is being completed by the applicant`s representative?
Do I tick one box only.
Very stressed and confused.
