Having been awarded chc for dad with not a considerable effort and help
One would hope the transfer from hospital would be timely and smooth

Not so records lost , meds missing , equipment missing ie bed sides

Please take as much info to the home as possible

The Nhs trust
The continuing health care team

Could either do a transfer safely with joined up thinking
And safeguarding

Day 1
Two falls , hanging out of his bed
Lack of knowledge and no care plan
Beware all

I'm really sorry, but I have had to remove the name of the PCT as per our T & Cs.

I am even more sorry that this has gone less than smoothly. Neither you nor your father need this. All I can suggest is that you complain to the appropriate authorities.

P.S. Even though my mother didn't have CHC there were two occasions where the NHS had to use a taxi service to send her medications to the home after she had been returned by ambulance.

Hello Tim, I am so sorry to read that the transfer did not go at all smoothly. I really hope your dad settles in soon. I would agree with your comment about giving staff as much info as possible, not just medical but all the little things too that will help your dad settle into his new home. The more info they have, the quicker and better they can get to know him.

Good luck to you both, this can still be a stress time. I hope you will have some time now to relax and do some of the things you want to do, but have some quality time with your dad.
