2nd time round activities coordinator,paperwork help

Hi

Im due to start in my workplace as activities co ordinator on Monday. Im bit stuck as I have totally forgot the way inwhich to get started regarding paperwork on each of my clients. Example: previous hobbies, risk assessment and most important record keeping of activities. Its really im a template I need some help with.

I know everything else will fall back into place such as the activitiy planning itself, but at the moment this is really baffling me.

Does anyone have any templates or advice that they would use.
