I'm not sure if it has changed but if someone is in a care home then the local CHC team arrange the assessment.
All residents should be reviewed periodically in the care home and be in receipt of Funded Nursing Care but if they deteriorate in the meantime you can request a CHC review.

There was a nurse assessor , a social worker and a member of staff at our recent meeting.

The nurse assessor is the one in charge. The one who takes the notes.

What's your mums behaviour like.
