My appointments (923) (previously My visitors book) has been updated and is now available to order. This booklet is designed to help people living with dementia keep track of the appointments they have with health and social care professionals.

There is space to record the professional's contact details, write down any questions ahead of the appointment, and make a note of what was said during the meeting.

What's new?
  • In this latest version, the booklet has changed title.
  • It now covers all appointments, not just home visits.
  • The format has changed from A5 to A4 to give the person more room to fill in the pages.
  • The booklet is now bright blue to make it easier to find and more identifiable as an Alzheimer's Society booklet.
  • There are new sections on what the booklet is for and how to use it, and there are quotes from people affected by dementia who reviewed the booklet.
  • The design of the pages has been changed so that it's more accessible and easier to use.
All of these changes were suggested by people affected by dementia.
