Register Enduring Power of Attorney COP42?

Hi,
We have reached the stage where we need to register my mother's EPA.
Me and my hubby do not wish to tell her since I think she would be very upset.
I think I will also ask her main carer for her opinion too.
If we do not notify my Mum - I think we need to refer to COP42 booklet but cannot find a copy online. Any one point us in the right direction please?
