Refused Continuing Healthcare

My mum has mixed dementia and refused Continuing Healthcare (CHC)
My evidence to support mum's case has been refused to be accepted at DST Assessment, Local Resolution and now Independent Review Panel. My evidence is well documented in care home notes and a video. I have followed the National Framework and Decision Support Tool to the letter and nowhere is it listed that evidence will be refused on grounds that it does not match the Local Clinical Commissioning Groups period of evidence. As long as evidence is relevant it should have been accepted. Can anyone confirm that I have to match the period of the CCG? I only found out about this fictitious period at the Independent Review Panel. This is unfair and the Chair has not been impartial by allowing CCG evidence and refusing mine. Can anyone answer if this is fair, whether my mother's human rights have been breached and what action can I take to get my evidence heard?
