my dad started with a generic social worker when he first had home care, and was transfered over to community mental health team when it became clear he had dementia type of difficulties. the actual transfer was a bit of a nuisance, but there were lots of positives. for one thing, the social worker was then in the same building as the CPN and the psychiatrist so communication was improved. lots of things happened to late to be of use (not to do with the social workers or the changes) but what was about to happened before he ended up in care was that he was going to get a separate specialist team of home care people rather than the generic ones who were more used to caring for people with physical disabilities. it would have got him much easier access to day care etc. so, on the whole, the change should be a good one.
