We’ve had a request for people to fill out this survey on people living with dementia in Extra Care Housing. This is a model of housing that supports independent living in self-contained accommodation, with 24-hour onsite flexible care provision, and access to a range of communal facilities.
Below are links to two online surveys. One is intended for completion by family carers of a person living with dementia in Extra Care Housing and the other is intended for people living with dementia (who may or may not live in Extra Care Housing). Surveys are anonymous and should take approximately 10 minutes to complete.
