COP 4 Form

Hi can anyone give me advice in completing a COP 4 Form I am stuck on section 5 5.1 and 5.2 does anyone have any idea?? It's so frustrating ?

When I applied to be mums deputy, section 5 on COP4 was just a personal statement about why I wished to apply - there were no subdivisions.
What do these sections ask?

Just looked, section 5 is a freeform statement of why the applicant wants to become a deputy.

Assuming COP4 is typo for COP1

5.1
A respondent is someone who has an interest in the application - usually the partner/parents/children of the person the application refers to, can be left blank if there are no respondents.
5.2
With regards people to notify, this would be people in order of closeness of relationship to the person the application relates to
