Complete refusal to wash

My mum has advanced mixed dementia and is now experiencing urinary incontinence. She will not change her clothes or allow herself to be washed. The situation is getting quite desperate. I'm looking for any advice as to what people have done in a similar situation. We've tried all the usual advice about routines etc. The GP says he would not sedate her so we can wash her.

Has anyone been in the same situation?
Many thanks.
