Hi everyone,

Our Policy team would like to hear from you. They're looking for feedback about care in hospitals, and would like to hear from carers, family and friends visiting a person with dementia or memory problems.

It's completely anonymous and if you'd like to have your say, please feel free to fill it in.

Between June and September the National Audit of Dementia (care in general hospitals) will collect feedback from carers on the care provided to people with dementia while admitted to hospital.

It will also be distributed by hospital staff to carers visiting throughout June, July and August. It can be returned directly to the Project Team in its attached freepost envelope (no individual responses will be viewed by hospital staff).

This questionnaire was used for the 2016 audit and received over 4600 responses. This gave hospitals scores based on the carer rating of care quality. These scores are now part of the summary information used by CQC in preparation for inspection. You can see the scores from the last round of audit here.

The questionnaire also includes a free text comment box for additional feedback. We would be very grateful for any comments, on good care, poor care, or suggestions for improvement. These will be included anonymously in local and national reports, with the scores and amalgamated feedback, and help hospitals to develop their plans for quality improvement.

There is a prize draw for one of five £50 vouchers for anyone completing the questionnaire. You will receive a postcard with your questionnaire form, or you will be redirected at the end of your online form.
