Advance care planning: your experiences

    Hi everyone,

    We've had the below request from our Dementia Voice team, who would like to hear from you about your experiences with advance care planning. Although the project is part of West Yorkshire and Harrogate, they feel that all responses (regardless of area) would be valuable.

    If this is something you're able to help with, please take a look at the survey if you have time. It shouldn't take long to fill in.

    We would value your input into this short survey which forms part of West Yorkshire and Harrogate Health and Care Partnership’s Advance Care Planning Pilot Project.

    Attached is an information sheet providing details about the purpose of the survey, and how your anonymised participation will shape a pilot resource around conversations for advance care planning.

    Advanced planning directives can be quite confronting. Our local hospital uses website www.myvalues.org.au which is a good starting point for those thinking about advanced care directives.
