Yes it can be done. 4 of our support group have managed it.

I am sure others here will help you. Good luck.

That's positive....long road ahead though, about to be admitted to specialist ward before all this can take place, don't really know the first thing about it though I'm sure like you all on here I'll be an expert when it's over....whenever that may be

My mother was awarded NHS Continuing care funding and my husband and I (with a team of care assistants from an agency) cared for her at home. The relevant case that made it possible for NHSCC to be provided for someone in their own home is that of Barbara and Malcolm Pointon. Best of luck with it.

Hi Gillybean
We got CHC funding for our Dad is his own home. Barb and I were looking after him 24/7 and had our own schedule worked out between us along with some private care so we could have a break.
NHS/CHC provided us with 7 morning calls and 7 back to bed, which was a godsend as our backs were just about ready to go with all the handling and lifting.
They also provided 4 waking nights (they said that was the maximum)
Dad was at a day centre 3 days and we just carried on with that.
Respite in a care home would have been paid for as well (but we couldn't get any!!!)

Here is the link for the up to date CHC forms.
The 'checklist' is completed first and if a person 'triggers' then a full CHC assessment is carried out.
Gathering evidence plays a crucial part in this process and the family should be encouraged to assist with this (this doesn't always happen).

We kept a diary of Dads dementia journey and this was actually used as evidence by his Community Matron at the assessment. We recorded, falls, changes in meds, hospital visits, chest infections, water infections etc.

Hope this helps
