We use a private care company for care visits for MIL (bathing, washing, changing clothes) As far as I can make out the company is not registered with the CHQ. Is there a requirement that all care companies are registered or are their exceptions?

They should be, yes. You can search CQC's website and look for details of provider registration or just give them a bell but all care providers (not privately employed individuals) need to be registered.
