getting to doctor office

Hi, my spouse has frontotemporal dementia. He was diagnosed 4 years ago. He is now inconvenience, and has frequent mood swings. He is in complete denial of his condition and refuses to get medical attention. I am trying to get him into a senior living center but must have a physician complete paperwork. Any suggestions are welcome!

From your wording I suspect you are resident in the USA, this makes it difficult to offer suggestions.

In the UK everybody is registered with a GP (general practitioner) which simplifies things.

Can you not take him to a physician to obtain the necessary paperwork?
The appointment could be for some kind of routine checkup.
