"Minimally able to reposition" = to an extremely small extent; negligibly. Meaning of negligibly: so small, trifling, or unimportant that it may safely be neglected or disregarded.
"Skin is generally intact" = skin is not completely intact.
"has limited sitting balance" = slumps when placed in seat
"limited function in legs" = cannot weight bear
"Can communicate verbally however not always reliably" = cannot communicate in a consistently good or accurate way.
"Mental Capacity Assessment suggest no mental capacity for complex decisions" = remove 'suggestion' from phrase.
"Paracetamol prescribed but rarely given" = individual does not have mental capacity or communication ability to say they have an headache
"There are no other significant care needs to be taken into consideration" = no assistance will be provided to address further needs.
Please feel free to add your own observations of phrases and/or words used by CHC Assessors. I have numerous examples to add - it would seem the mis-use/misunderstanding of words by CHC Assessors is widespread. So are the descriptions within domain level of need.
