Has your Mum had a fall, my Mum used to fall at home and get black eyes. She could also have bumped into something.
Suggest you make a note of date and anything else you can think of around the situation and then monitor to see if it happens again and if it does try and see if there is a link.
I'd be a bit worried that no-one had told you even if they didn't know the reason for it. I'd accept that these things happen but would just like to know that someone cared that it had.
I would say it is most likely she fell or bumped in to something but i would be worried that the home didnt call you right away to tell you. You'd be right to ask if no one knows how she got it, why werent you informed anyway. I check my dad frequently and hes had various bruises cuts etc and i question every single one. To their credit, if my dad falls or has a wound for whatever reason they call me immediately and if im happy with their report i let it go. I would say use caution and check if you think somethings not right and never be afraid to speak up.

Yes record it for any future reference and request that the Manager carries out a full investigation.
