CHC process: confused about which panel etc?

I'm sorry if I'm just being dense (very likely!) but I'm feeling increasingly confused about the CHC process we've just started and although I've read the screening, decision tool and framework stuff I can't get to grips with it. Hoping someone who's been through it can...

Mum's been in hospital for about three weeks after a fairly sudden deterioration, though struggled on for years before that. Hospital transfer team mentioned CHC assessment. Mum was screened as possible and now we have a meeting with nurse, social worker etc next week to go through the decision tool (I think). Then they mentioned taking it to panel before our meeting for local authority funding, after which it would go to the CHC panel for decision. I don't understand what the local authority panel is as I thought if its CHC it's all NHS. Can anyone help so that I at least feel informed about what might be going on? Thanks

When we were going through the process I found this Factsheet from AGE UK the best explanation of the process, with links to the legislation.
