			<description><![CDATA[Visited Brian after work today, his one to one worker was with him, he was a bit agitated but had walked to lounge earlier with one carer's help so improvement with mobility. He had sandwiches for his tea and managed them himself, another improvement. He was trying to get out of chair so carer...]]></description>
			<content:encoded><![CDATA[<div>Visited Brian after work today, his one to one worker was with him, he was a bit agitated but had walked to lounge earlier with one carer's help so improvement with mobility. He had sandwiches for his tea and managed them himself, another improvement. He was trying to get out of chair so carer asked if he needed the toilet, he said yes but then sat back in chair. Bit later another carer came in and Brian looked relieved and again tried to get up, she helped him up and got him onto commode, he is wearing pads at the moment and had already soiled it, carer cleaned him up and said he was a bit sore so put some cream on and put him in his pyjamas ready for bed. He seemed relieved to be back on his chair and looked tired out from the effort, I left him to settle down and then go to bed. Home manager said she had asked district nurse about a hospital bed but she said he couldn't have one as he did not have bed sores and was not on her list. Manager said he was incontinent and therefore needed to be put on her list so we will see! why is everything a battle!</div>
